Measuring Confidence for Community-Based Childbirth in the United States: A Mixed Methods Study with CHOICES Memphis Center for Reproductive Health

Fear during pregnancy and birth can worsen health care experiences and outcomes, especially among populations experiencing racism, disrespect and reproductive inequities. Far less is known about an important counterpart, confidence. This dissertation aimed to elucidate the pathways through which midwifery-led care impacts prenatal confidence for physiologic birth and to inform future care, centering the perspectives of Black populations. The work includes an instrument adaptation resulting in the Preparation for Community-based Labor and Birth (P-CLAB) to measure confidence for community-based birth, a mixed methods study of prenatal confidence from the perspective of birth center patients and providers, and the qualitative and psychometric evaluation of the P-CLAB.
